The NCD program reached 51 people through community awareness activities and conducted static outreaches including 18 screenings for hypertension, diabetes, and other non-communicable diseases. A total of 17 new cases were identified and linked to care, while 13 patients received follow-up support and treatment services.
The program also trained 2 health workers and conducted 01 mobile outreach event across target communities. These efforts contributed to increased access to early detection services and strengthened continuity of care for individuals living with NCDs.
Key priorities for the coming month include expanding community screening coverage, improving patient follow-up, and strengthening referral systems to enhance NCD prevention and management outcomes.
